Service Description

Begins with evaluation/discussion of client's life history and interests to identify the daily activities most important to the client that they may be having difficulty performing or establishing. Client and OT will then collaborate to develop goals and a personalized intervention plan for improving participation in these daily activities.

Cancellation Policy

Please provide a minimum of 24 hours advance notice for cancellations and rescheduling requests.
